<br /> The San Joaquin County Environmental Health Department (EHD) has received and reviewed
<br /> Work Plan for Well Destruction, dated 30 May 2014, prepared and submitted by your consulting
<br /> firm, Stantec Consulting Services, Inc. (Stantec), on your behalf for the above-referenced site.
<br /> In Work Plan for Well Destruction, Stantec proposed destruction of a total of 12 wells. Based on
<br /> recent groundwater data and residual soil impact, Stantec proposed to destroy monitoring wells
<br /> MW-3, MW-4, MW-8 and oxygen injection wells, 1-4 and 1-5 by over drilling and removing all well
<br /> contents (conductor casing, well casing, screen, filter pack, grout sealant). Stantec proposed to
<br /> destroy the remaining wells, MW-1, MW-5, MW-6, and MW-7 and oxygen injection wells 1-1, 1-2,
<br /> and 1-3 by pressure grouting. The pressure grouting method briefly consists of filling the well and
<br /> filter pack with neat cement grout through a tremie pipe from total depth to top of well casing.
<br /> Grout will settle and then topped off before pressure is applied to the wells at 25 pounds per
<br /> square inch for ten minutes. The proposed well destruction through pressure grouting is approved
<br /> by EHD except for monitoring well MW-6, due to residual methyl-t-butyl-ether (MTBE) in
<br /> groundwater. Monitoring well MW-6 must be over drilled along with the wells previously proposed
<br /> for over drilling. In the work plan, Stantec proposed to submit a Well Destruction Report after all
<br /> wells have been destroyed summarizing the field activities. In addition to the relevant data,
<br /> include properly signed manifests for all disposed water/soil waste related to the destruction of the
<br /> wells at this site.
<br /> Stantec proposed to use neat portland cement as the sealing material for the wells. All water
<br /> used for the purpose of sealing a well must be potable water and not water that has been
<br /> displaced from a well that is.to be destroyed. With these conditions, the EHD approves the work
<br /> plan. Please submit a completed Mast r�vi1q Record form, a Well and Boris yrDVt Application ,���✓
<br /> form, and a permit/inspection fee of $�o5-80 (permit fee will increase to $1,,145.00 as of August
<br /> 1, 2014). Notify the EHD for inspection of all destruction of wells at least two weeks prior to
<br /> destruction activities. The EHD will issue a closure letter once and all wells have been properly
<br /> destroyed and appropriately signed waste manifests have been submitted to this agency.
