11. Visual contact must be maintained between pairs on-site with the team <br /> members remaining in close proximity in order to assist each other in case <br /> of emergencies. <br /> 12. The number of personnel and equipment in the contaminated area should <br /> be minimized consistent with site operations. <br /> 13. Appropriate work areas should be established for support, contamination <br /> reduction, and exclusion areas. <br /> 14. Establish appropriate decontamination procedures for leaving the site. <br /> 15. Report all injuries or illnesses, unsafe conditions, practices or equipment <br /> immediately to the site safety coordinator. <br /> 16. A portion of the site "field book" will be maintained as a project safety log. <br /> The project safety log will be used to record the names, entry and exit dates, <br /> and time of all HSI GeoTrans, Inc. and subcontractor personnel, and of <br /> project site visitors; air quality and personal exposure monitoring data; and <br /> other information related to safety matters. All accidents, illnesses or other <br /> incidents shall be reported immediately to the Operations Manager and to <br /> the HSI GeoTrans, Inc. Health and Safety Officer. <br /> 2 <br />
