COMPLAINANT STATED THE FOLLOWING DAY WAS ILL WITH SYMPTOMS OF DIARRHEA, <br /> HEADACHE,ABDOMINAL PAIN, LOW GRADE FEVER AND CHILLS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> Walk in cooler--41.00°F Salad prep line--41.00°F <br /> Steam table--150.00°F <br /> NOTES <br /> This is a complaint inspection <br /> -Management unaware of complaint. <br /> -Manager states only 1 employee called in sick 03/21/2018. Reviewed the employee sick log. The employee who called in <br /> sick yesterday was not working on 03/19/2018. <br /> -Manager states no employees have been out of the country in the last 2 weeks <br /> -Manager states employees do not work in a health care facility <br /> -Observed an employee make a chicken salad: <br /> -Classic salad comes with greens,tortilla chips, cotija cheese, corn, diced chicken, and salsa <br /> -The greens, cotija cheese, and salsa are at 41 F. The corn and diced chicken are at 150 F <br /> -Observed employee perform proper hand washing at the kitchen hand sink prior to putting on gloves and making the salad <br /> -Employee used the gloved hands to place the greens,tortilla chips, and cotija cheese. Employee used scoops to place the <br /> diced chicken and corn on the salad-The greens come to the facility prewashed. All other items come to the facility <br /> prepackaged. <br /> -The salsa is made on site at the prep sink.The salsa ingredients are tomatoes, cilantro, salt, pepper, and serrano peppers. <br /> The prep sink and food prep equipment was clean. <br /> -Sanitizer bucket: 300 ppm QUAT.
