To proceed with the closure of this site, please submit to PHS/EHD a well permit <br /> application for destruction of the onsite monitoring well along with the required $60.00 <br /> permit fee. <br /> In addition, please provide to PHS/EHD certification in writing that all record fee title <br /> owners have been notified of your request for closure. Section 25297.15(a) of Chapter <br /> 6.7 of the Health and Safety Code requires that a local agency not issue a closure letter, <br /> or make a determination that no further action is required with respect to "hazardous <br /> substance" underground storage tank release sites unless all current record owner of fee <br /> title to the site have been notified of the proposed action by the primary or active <br /> responsible party. If you are the sole property owner, provide a statement of that fact to <br /> PHS/EHD to fulfill this requirement. Please submit this information as soon as possible <br /> so that the closure process is not delayed by this requirement.
