The State Water Resource Control Board(SWRCB)will invoice you directly for activities performed
under the auspices of the Local UST Oversight Program. The goal of the site evaluation performed by Public Health
Services Environmental Health Division (PHS/EHD) is to certify closures of underground tank leakage sites.
Closure certification occurs only after mitigation activities are completed.
In addition, the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground
Tanks" and "Appendix A" reporting format and the LUFT Manual shall be utilized as guidance documents in the
preparation of work plans and/or reports.
In previous correspondence San Joaquin County Public Health Services,Environmental Health Division(PHS/EHD)
notified you that further site investigation was needed and that sites would be directed to begin their investigation
based on priority ranking. Due to the high soil contaminant levels and the shallow depth to groundwater at the site,
you are now being directed to begin your investigation. In order to remain in compliance with PHS/EHD directives,
a work plan to continue the soil and`or groundwater investigation should be submitted by December 1, 1994. ,
If you have any questions or there are any corrections in the responsible party information, contact Mary Meays,
Senior REHS at (209)468-0337.
Donna Heran, REHS, Director
Environmental Health Division
zl' 1 w
Mary/ Meays, Senior REHS Diane*MHinson, REHS
Site Mitigation Unit Supervisor
MNV1600SITE
Enc.
c: SWRCB, Underground Storage Tank Section - Lori Casias
c: CRWQCB, Central Valley Region - Elizabeth Thayer
`I()i.i.ion of San Joaquin County Health Care Service.v%mme,
