The consultant must have at least 5 years experience con- ducting underground storage tank corrective action work in California. A statement of qualifications attesting to such experience must accompany any bid submitted. 2 . It is preferred, but not required, that the consultant have an office within 30 miles of the corrective action site. 3 . The consultant must employ qualified persons to perform the work described above, including licensed geologists and other technical professionals that possess all necessary licenses to operate within the State of California and the jurisdiction of San Joaquin County. A list of qualified persons who will perform the work must accompany any bid submitted. 4 . The consultant must carry out the described work in a time- ly, professional, safe, and legal manner and maintain neces- sary records and other safeguards to ensure that reported sample results are accurate. All work must be performed in a manner acceptable to the PHS/EHD. 5 . The consultant must provide at least 3 reference(s) of pre- vious clients for whom the consultant has performed similar work within the past 2 years . 6 . The consultant must maintain general liability insurance in the amount of $1,000,000 . 00 and professional errors and omissions liability insurance in the amount of $1, 000, 000 . 00 for the life of the project. Ruiz Grocery shall not be responsible for the cost of any associated premiums . Con- sultant shall be responsible for the cost of any associated premiums . Consultant shall be responsible for any third party claims related to the execution of work described herein, with the exception of claims filed by Ruiz Grocery employees . Proof of such .insurance shall be furnished to Ruiz Grocery prior to the start of work. 7 . Work shall be performed in a manner which will not require the cessation of business at the site unless absolutely necessary or dictated by the PHS/EHD. 8 . Consultant shall provide a copy of consultant 's standard services agreement and schedule of charges with any bid submitted. The schedule of charges shall include: (a) hourly rates for personnel classifications employed by con- sultant; (b) unit charges for common supplies and equipment used in corrective action work; and (c) multipliers used when charging any other direct expenses (e.g. subcontrac- tors) . —3— 002753.erre/=CMKB 60410-1B061/H103/03/27/9..
