Bypassing of pretreatment facilities at any time is <br /> strictly prohibited without the express written consent of <br /> an authorized City of Stockton representative in advance. <br /> 6. Any breakdown or interruption of pretreatment, or any <br /> accidental discharge to the City of Stockton storm or <br /> sanitary system must be reported immediately. Non- <br /> reporting of spill or slug incidents will be cause for <br /> administrative permit review and/or revocation of Permit. <br /> 7 . Permittee is required to self-monitor wastewater generated <br /> on site for pollutants specified by the City of Stockton. <br /> Sampling procedures and handling must be approved by the <br /> City of Stockton and the laboratory utilized must be <br /> certified by the State of California Department of Health <br /> Services for the specific analysis conducted. Sampling <br /> frequency and reporting will be determined on a "case-by- <br /> case" basis, but must at least comply with 40 CFR 403 . 12 <br /> (g) "Monitoring and Analysis to Demonstrate Continued <br /> Compliance" and any specific categorical requirements. <br /> 8 . Permittee shall perform all required quality assurance and <br /> control procedures as contained within Attachment Quality <br /> Assurance Manual. <br /> 9 . If more than the required number of samples are taken for <br /> analysis, those results shall also be reported to the City <br /> of Stockton. Permittee must report all results (40 CFR <br /> 403 . 12 (k) (1) . <br /> 10. The City may use grab samples for the determination of <br /> compliance with local limitations and other applicable <br /> limits. <br /> 11. The permittee must notify the City of Stockton, <br /> Environmental Control Office at 944-8759, within 24 hours <br /> of discovery of any violation with Federal categorical <br /> Pretreatment Standards, State, local limitations or any <br /> condition contained within this permit. permittee shall <br /> repeat sampling and pollutant analysis for those which <br /> exceeded limitation requirements and shall submit, in <br /> writing, the results of this second analysis within 30 <br /> days after becoming aware of this violation. The report <br /> shall also include all steps taken or to be taken to <br /> reduce, eliminate, and prevent recurrence of such <br /> violations. <br /> 12 . Permittee shall comply with all reporting requirements <br /> contained within the Federal categorical regulations (40 <br /> CFR 403 . 12) . At a minimum, unless otherwise specified in <br /> your wastewater discharge permit, semiannual reports must <br /> be submitted to the City by January 10th and July 10th <br /> (for the previous six month period) . <br /> 13 . Information and data on a permittee obtained from reports, <br /> questionnaires, permit applications, permits and <br /> monitoring programs and from inspections shall be <br /> -2- <br />
