During the monthly inspection,the <br /> designated operator shall review the alarm history for the previous month, check that each alarm was documented <br /> and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a repeat violation, Class II. <br /> 210 CCR 2716(f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1, 2018). <br /> The September 2018,July 2018, and April 2018 designated operator monthly inspection reports were not found on <br /> site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. <br /> Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 301 HSC 25292.1(a) UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Fuel filters were observed outside of all under dispenser containments at the facility.The underground storage tank <br /> system shall be operated to prevent unauthorized releases. Immediately contact a properly licensed,trained, and <br /> certified contractor to replace or upgrade the component under permit and inspection with the EHD. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The Bravo box float and chain in the Diesel#2 UDC sump failed to stop the flow of product at the dispenser when <br /> tested. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of <br /> product at the dispenser when it detects a leak. The service technician adjusted the chain(s)and verified <br /> functionality. <br /> This is a repeat violation, Class II. <br /> 320 CCR 2635,2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Owners or operators of underground storage tanks that do not meet California Code of regulations,title 23 division <br /> 3, chapter 16 section 2635(c)(2)shall test overfill prevention equipment once by October 13th, 2018 and every 36 <br /> months thereafter; and within 30 days of the date of a repair.The site did not performed the testing by October 13th, <br /> 2018. Ensure to conduct overfill prevention equipment inspection within the time frame provided by the regulation <br /> and submit test results within 30 days of performing the inspection.
