The following is an itemized list of underground storage tank violations that have not <br />been addressed for CHEROKEE LANE SERVICE STATION* as of December 18, <br />2014. <br />Open violations from October 28, 2014 inspection <br />Violation #207 - Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the February 2014 and <br />the September 2014 designated operator monthly inspection reports and failed to check that they were responded to <br />appropriately. The missing alarms include: diesel fuel alarm triggered during the 2/7/2014 re -inspection, and no alan <br />record was attached to the September 2014 designated operator inspection report. During the monthly inspection, <br />the designated operator shall review the alarm history for the previous month, check that each alarm was documents <br />and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br />including all of the required information on the reports. This is a Class II violation. <br />Violation #208 - Designated operator monthly inspection reports not maintained on site for at least 12 <br />months. <br />The October and November 2013 designated operator monthly inspection reports were not found on site. Designat( <br />operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that <br />copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. <br />Submit copies to the EHD. This is a Class II violation. <br />Violation #310 - Sensor in a turbine sump failed to activate an audible and visual alarm or stop the flow <br />product. <br />The 87A and 91 -octane sensor failed to activate an audible and visual alarm at the panel when tested to ensure <br />continuous monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a <br />properly licensed, trained, and certified contractor replace the failed component with a functional component (LG 11 <br />listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be <br />replaced immediately, there is a possibility that the 87A and 91 -grade UST system may be red tagged to prevent fuel <br />inputs.
