Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by 12-1-11. <br />105. UST diesel, 91, 87A(west), and 8713(east) tank forms on file with the EHD are not current. Any <br />change of information on these forms must be submitted to the EHD within 30 days of the changes. <br />Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate <br />tank form for each tank. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site. The <br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br />of the monitoring and response plans for approval by the EHD by 12-1-11. An approved copy will be <br />returned for your records. <br />112. The positive shutdown for the power out/sensor out (fail safe) of the submersible turbine pump <br />sensors was not able to be verified on 1-26-11. Part way through the inspection, the diesel turbine sump <br />would not turn on for positive shutdown to be verified. It could not be verified that this facility meets the <br />requirements for annual line tightness testing exemption. The line test was not conducted. Annual line <br />tightness testing is required unless the UST system is equipped with the following-. <br />1. A continuous monitoring system that either activates an audible and visual alarm or stops the flow <br />of product at the dispenser when it detects a leak <br />2. A monitoring system that shuts down the pump or stops the flow of product at the dispenser when <br />it detects a leak <br />3. A monitoring system for piping other than that in UDC sumps (tank top sumps, transition vents, <br />etc.) is failsafe and shuts down pump when a leak is detected <br />During the inspection on 10-31-11, power out/sensor out(fail safe) was verified. <br />203. Approximately one piri�of diesel was observed in the diesel submersible turbine pump sump <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT END'S CURRENT HOURLY RATE. <br />Inspector.
