Lodi CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />operator shall train facility employees for which he or she Is responsible lsible in the proper operation lI and <br />maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and <br />submit a copy of the training records to the EHD by 12-1-11. <br />306. A fuel leak was found at ancillary equipment in the diesel submersible turbine pump sump. All <br />primary containment must be product tight. Have a properly licensed, trained, and certified contractor <br />trouble shoot, replace, or repair the leaking components. A permit and inspection by the EHD may be <br />required. If the leaking component can't be repaired or replaced immediately, there is a possibility that <br />the diesel UST system may be red tagged to prevent fuel inputs. <br />Note: Part way during the inspection, the diesel turbine could not be turned on. A functionality test of the <br />diesel mechanical line leak detector was not performed during the inspection. Immediately schedule this <br />test with the EHD to be performed. The re -inspection may be billed at the EHD's current hourly rate. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation, by 12-1-11. <br />Note: The return to compliance certification documents for the 1-26-11 monitoring system certification <br />and routine inspection have not yet been received. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />EHD Inspector: <br />Aris Cacapit (209) 468-3453 <br />Received Bry: <br />Title: <br />EHD 23-02-003 Rev 07/12111 CONTINUATION FORM <br />
