Any change of information on these forms must be submitted to the EHD within 30 days of the changes. Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate tank form for each tank. #4 Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents are required to be submitted annually. Complete and submit a copy of the financial responsibility immediately. #5 & 6 Current approved copies of the monitoring and response plans were not found on site. The monitoring and response plans must be current and approved by the EHD. Complete and submit a copy of the response plan for approval by the EHD by 11-27-10. An approved copy will be returned for your records. #11 The 87A (west) stp and diesel stp sump sensor were approximately 3 inches higher than the lowest point of the sump almost and not properly located to detect a leak at the earliest opportunity. The 91 stp sump sensor was laying on its side and not properly located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Technician corrected this during the inspection by repositioning the sensor to the lowest part of the stp sump. #21 Liquid was observed in the south diesel satellite udc. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. Technician removed this liquid and placed into the hazardous waste drum on site. Ensure that the dispenser udc's and tank sumps are maintained free of liquid. #23(c) On the November 2010 designated operator report, the designated operator failed to check and note the correct test dates for annual monitoring system certification testing. The annual monitoring system certification test date was documented as 10-27-10 but should have been documented as 10-6-09. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
