An approved copy will be returned for your records. 109. Annual monitoring system certification, leak detector testing, and spill container testing were performed on 10-31-11 and a test report was not submitted to the EHD until 7-11-12(six months late). A copy of the test report must be submitted within 30 days of the tests. Ensure that you submit a copy of the test reports to the EHD within 30 days of the tests. 205. The October 2011, November 2011, December 2011, January 2012, February 2012, March 2012, June 2012, July 2012, August 2012, and September 2012 designated operator monthly inspection reports were not on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated monthly inspection reports are maintained on site. Submit copies to the EHD by 11-29-12. 310. A small amount of liquid was found in the #9 satellite dispenser sump sufficient to raise the bravo float but not enough to trip the shear valve. If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. During the inspection the ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
