During the monthly inspection,the designated <br /> operator shall review the alarm history for the previous month, check that each alarm was documented and <br /> responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are <br /> including all of the required information on the reports. <br /> This is a minor violation. <br /> 210 CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br /> The July 2017 through September 2017 designated operator monthly inspection reports were not found on site. <br /> Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate <br /> and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copies to the EHD. <br /> This is a Class II violation. <br /> 309 CCR 2636(f)(1) Monitoring in sump/UDC failed to activate an audible and visual alarm or stop the flow product. <br /> The 87-octane float and chain in UDC 1/2 failed to stop the flow of product at the dispenser whent tested. All <br /> monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the <br /> dispenser when it detects a leak. The 87-octane float and chain in UDC 1/2 was adjusted, retested,and passed. <br /> This was corrected on site. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Approximatley 2 cups of liquid was observed in the diesel subermsible turbine pump(STP)sump. If water could <br /> enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br /> and manage it accordingly. Ensure that the diesel subermsible turbine pump(STP)sump is maintained free of <br /> liquid. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 11/25/17 . <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer(if <br /> FA0000556 PR0231841 SCO01 10/26/2017 <br /> EHD 23-01 Rev.09/22/16 Page 5 of 6 Underground Storage Tank Program OIR <br />
