Current financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Complete and submit a copy of the <br />financial responsibility by July 18, 2011 <br />105. UST diesel, 91, 87A(west), and 8713(east) tank forms on file with the EHD are not current. Any <br />change of information on these forms must be submitted to the EHD within 30 days of the changes. <br />Immediately complete and submit a copy of the facility and tank forms to the EHD. Complete a separate <br />tank form for each tank. <br />106 & 202. Current approved copies of the monitoring and response plans were not found on site nor on <br />file with the EHD. The monitoring and response plans must be current and approved by the EHD. <br />Complete and submit a copy of the monitoring and response plans for approval by the EHD by July 18, <br />2013. An approved copy will be returned for your records. <br />108. Secondary containment testing was performed on October 18, 2012, and a test report was <br />submitted to the EHD on May 21, 2013(seven months late). A copy of the test report must be submitted <br />within 30 days of the test. Ensure that you submit a copy of the test reports to the EHD within 30 days of <br />the tests. <br />109. Annual monitoring system certification, leak detector testing, and spill container testing were <br />performed on October 29, 2012, and a test report was not submitted to the EHD until May 28, 2013 <br />(seven months late). A copy of the test report must be submitted within 30 days of the tests. Ensure that <br />you submit a copy of the test reports to the EHD within 30 days of the tests. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector.
