Cherokee Lane Lodi, CA 95240 <br />PROGRAM COORDINATORS <br />Robert McClellon, REHS <br />Jeff Carruesco, RENS, RDI <br />Kasey Foley, REHS <br />Linda Turkatte, REHS <br />San Joaquin County Environmental Health Department (EHD) performed a routine underground storage tank (UST) <br />inspection on October 31, 2011, reinspections on February 14, 2012, and March 21, 2012, and the most recent <br />routine UST inspection on October 29, 2012 at the above referenced site. An inspection report was issued <br />identifying information to be submitted to bring this site into compliance. This information was required to be <br />submitted by within 30 days of the inspection. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($125).
