The following is an itemized list of underground storage tank violations that have not <br /> been addressed for CHEROKEE LANE SERVICE STATION` as of June 08, 2017. <br /> Open violations from October 17, 2016 inspection <br /> Violation#103 -Current financial responsibility documents not submitted. <br /> Financial responsibility documents have not been submitted to the EHD. The finacial resposibility documents are <br /> outdated. Current financial responsibility documents are required to be submitted annually. Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required <br /> documents. <br /> Violation #104- UST operating permit application for facility and tank information not submitted or current. <br /> UST Tank Information forms for the87A, 8713, 91 are not current in CERS. The secondary containment for the vent <br /> and vapor recovery piping should be marked none for each of these tank systems. Any change of information must <br /> be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, <br /> and submit for review by the EHD. <br /> Violation#105 -Failed to have an approved UST monitoring plan. <br /> The monitoring plan is not current. The 87A 12,000 gallon UST system and the diesel 10,000 gallon UST system <br /> should each list VMI as the mechanical line leak detector manufacture and LD 99-2000 as the model. The <br /> monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into <br /> CERS, make the necessary changes, and submit for review by the EHD. <br /> Violation#107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br /> performed. <br /> An accurate UST Monitoring Site Plan was not submitted. The site plan uploaded into CERS on 7-14-14 does not <br /> list the mechanical line leak detectors, the float and chain leak detection mechanism within the UDC sumps, nor the <br /> Incon monitoring panel. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload <br /> a copy of the UST Monitoring Site Plan. <br /> Violation#121 -Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on October 12, 2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance. This information was required to be submitted by November 12, 2915. <br /> This information has not been received resulting in a non-compliant status for this facility. An operator that receives <br /> an inspection report shall have 30 days to submit a written response that includes a statement documenting <br /> corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response <br /> documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> Violations still Open: <br /> - HSC 25292.2 Current financial responsibility documents not submitted <br /> - CCR 2711 UST Operating Permit Application for Facility and Tank information not submitted or current <br /> - CCR 2632 Monitoring and Response Plan not current or approved by EHD <br /> - CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed <br /> CCR 2638(d) Failed to submit monitoring system certification test report within 30 days <br /> Violation#204-Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be <br /> accessible on site at all times. Immediately locate and retain a copy on site. <br /> Violation#210 -Designated operator monthly inspection reports not maintained on site for at least 12 <br /> months. <br /> The September 2016 designated operator monthly inspection reports were not found on site. Designated operator <br /> monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies <br /> of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit <br /> copies to the EHD. <br /> Page 1 of 2 <br />
