An inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by 2-26-11. This <br />information has not been received resulting in a non-compliant status for this facility. <br />Note: the sensor out/power out (fail safe) tests to verify positive shutdown of the turbines could not be verified <br />during the 1-26-11 annual monitoring system certification testing. Per the technician equipment failure that occurred <br />during the inspection prevented the diesel turbine from turning on. Immediately have a licensed certified contractor <br />trouble shoot and repair the equipment. A permit may be required depending upon he scope of work. Immediately <br />schedule an inspection to verify sensor out/ power out (fail safe) tests to verify positive shutdown of turbines or <br />schedule for tank line tests to be performed. Please be aware that a re -inspection will be charged at our current <br />hourly rate. The 1-26-11 annual monitoring system certification is still considered incomplete. <br />Failure to correct the violations will result in continuing/additional violations. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />current hourly rate ($122). Failure to submit the completed information immediately may result in a re- <br />inspection or further legal action. <br />The missing information includes: <br />F Current UST Facility form (former Form A) <br />r Test results from the Annual Monitoring <br />Certification of positive turbine shutdown for <br />fail safe and power out or annual line test <br />P-0, Response plans <br />we, Certification of Financial Responsibility and <br />Letter from Chief Financial Officer <br />lv, Certification of Return to Compliance (Signed <br />and dated) <br />V Current UST Tank form (former Form B) <br />r!
