Designated operator monthly inspection reports for the previous twelve months shall be <br />retained on site. Locate and ensure that copies of the previous twelve months of designated operator <br />monthly inspection reports are maintained on site. Submit copies to the EHD by 2-26-11. <br />#23(c) The designated operator failed to attach the alarm history report for the previous month on the <br />designated operator November 2010 monthly inspection report. During the monthly inspection, the <br />designated operator shall review the alarm history for the previous month, check that each alarm was <br />documented and responded to appropriately, and attach a copy of the alarm history with documentation <br />taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />24. The annual monitoring system certification testing was last performed on 10-15-09 and was due by <br />October 2010. Testing was performed on 10-27-10. However, the technician David Winkler could not <br />submit a current copy of his Incon level 2 certification. This facility has an Incon TS 5000 monitoring <br />panel for the UST system. David Winkler did not obtain a current Incon level 2 certification until <br />12-13-10, one month and 16 days after performing the monitoring system certification. The monitoring <br />system certification testing that David Winkler performed cannot be accepted. Zane Nimmo returned to <br />the site on 1-26-11 to perform the annual monitoring system certification, leak detector, and spill <br />container testing. The required monitoring system certification testing for this facility is considered 3 <br />months late. These tests are required once every 12 months. Continue to ensure that the required tests <br />are scheduled on time and provide 48 hours notification to the EHD. Please be aware that today's re- <br />inspection will be charged at our current hourly rate. <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentation by 2-26-11. <br />Note: The sensor out/power out (fail safe) tests to verify positive shutdown of the turbines could not be <br />verified today. Per the technician equipment failure that occurred during the inspection prevented the <br />diesel turbine from turning on. Immediately have a licensed certified contractor trouble shoot and repair <br />the equipment. A permit may be required depending upon the scope of work. Immediately schedule an <br />inspection to verify sensor out/power out (fail safe) tests to verify positive shutdown of turbines or <br />schedule for tank line tests to be performed. Please be aware that a re -inspection will be charged at our <br />current hourly rate.
