The requested information was required to be submitted by <br /> November 22,2008 and still has not been received resulting in a non-compliant status for this facility. <br /> Failure to correct the violations will result in continuing/additional violations. <br /> Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is billed at the <br /> current hourly rate($105).
