The <br />permit has been approved with the following conditions: <br />1. Contact Lori Luces at (209) 468-0341 after a determination has been made to which <br />device will be installed. Approval of the device must be given prior to installation. <br />2. Any changes in information or scope of work must be submitted to SJCEHD prior to <br />commencing work. <br />3. At least one person who holds the International Code Council "UST <br />Installer/Retrofitter" certification must be onsite AT ALL TIMES when work is <br />performed. <br />4. Contact Lori Luces at (209) 468-0341 to arrange the following inspection <br />appointments; a 48-hour notice for inspection appointments is required. <br />a. Leak Detector: Must function to detect a 3 -gallon per hour leak at 10 psi. <br />5. Permit expires 90 days from the approval date. Note: if an extension to the <br />expiration date is required, this office prior to the expiration date must receive a <br />written request. <br />Thank you, <br />Lori J.
