Please submit the following <br />within 30 days: <br />* 1.Current State Form B for each tank located on your property. <br />*2. Financial Responsibility and Letter from the Chief Financial Officer. <br />*3. Monitoring and response plan <br />4. Annual monitoring certification results and line leak detector results. <br />*Copies of each of these documents are enclosed for your convenience. <br />In addition, there is a hazardous waste generator located on your premises. A green <br />owner information form was left with the operator of this business on May 24, 2001. That <br />green form has not been returned to this office. It is the responsibility of the property <br />owner to ensure that the businesses located on his property are operating in accordance to <br />the law. A new green form is enclosed.
