This information has not been received resulting in a <br /> non-compliant status for this facility. <br /> Failure to correct the violations will result in continuingladditional violations. <br /> Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is billed at the <br /> current hourly rate($98).
