This information has not been received resulting in a <br /> non-compliant status for this facility. <br /> Failure to correct the violations will result in continuing/additional violations. <br /> Note: All EHD staff time associated with failing to comply,including the issuance of this letter,is billed at the <br /> current hourly rate($98). Failure to submit the completed information immediately may result in a re- <br /> inspection or further legal action. <br /> The missing information includes: <br /> A signed certification of return to compliance(Back of page 1 of the inspection report). Disposal receipts and/or <br /> analytical test results for the following items: contents of the blue fifty-five gallon drum that was located underneath <br /> a trailer and then moved to the hazardous waste trailer,the metal shavings that were removed from the location <br /> behind the shop,the contaminated soil removed from underneath the sweeper and the contents of the container <br /> underneath the parts washer in the shop that is no longer being used. Documentation that all containers with <br /> hazardous wastes are labeled properly and kept closed except when adding or removing hazardous wastes. <br /> Documentation that the spill control equipment has been maintained properly(spill pallet is free of liquid). <br /> Most forms may be accessed from our web site address,www.peocities.com/unitiii.
