El Dorado St. 1080-2 <br /> Stockton 95206 <br /> zl <br /> The San Joaquin County Public Health Services, Environmental Health Division <br /> _ has recently been included in the State Water Resources Control Board (SWRCB) € <br /> UST Pilot Program for oversight of assessment and clean up activities at <br /> contaminated underground tank sites. <br /> A proposal for initial assessment and/or remediation .#or the above referenced <br /> site has been submitted to this office. <br /> f All Proposals and reports submitted must be accompanied by a Letter of <br /> Acknowledgement From the respoasible party(ies) which states, at a minimum, <br /> the following: <br /> I(We) dtclare, under penalty of perjury, that the information <br /> and/or recommendations contained in the attached proposal or <br /> t report is/are true and cor,:ect, and that all work and reports <br /> which required geologic or engineering evaluations and/or <br /> judgments have been performed under the direction of an <br /> appropriately registered or certified processional. <br /> The attached proposal or report utilizes the "Regional Board Staff <br /> Recommendations For Initial Evaluation and Investigation of <br /> Underground Tanks" and "Appendix A" reporting format.
