Contaminated groundwater pumped out of the excavation <br /> shall be containerized for proper disposal. <br /> • This site has a number of wells located off-site in the City of Manteca right-of-way. <br /> Please submit a copy of the encroachment permit issued by the City of Manteca <br /> when submitting the EHD Well and Boring Permit Application form for these wells. <br /> • EHD records indicate the presence of four 'nested' wells [MW-1/MW-1A; <br /> MW-4/MW-4A); MW-5/MW-5A; and SP-1/SP-1A] installed using eight-, ten- and 12- <br /> inch diameter augers. The EHD has found destruction results of nested wells are <br /> best obtained when augers larger in diameter than the original augers are used and <br /> centered on the deepest casing (which is off-center in the original borehole). The <br /> auger needs to be sized sufficiently to reach out and remove all well casings, <br /> screens, filter packs, and grout sealant installed during construction. <br /> After the EHD has approved the well destruction scope of work proposal and the public <br /> participation comments have been properly addressed, you may submit an EHD Well and <br /> Boring Permit Application form along with a Master File Record form, encroachment permit <br /> issued by the City of Manteca and appropriate fees for each parcel. The permit fee for the <br /> destruction of each well is $125, along with a $375 Inspection Activity fee per parcel. A <br /> review of the site records indicate that eight monitoring wells, three oxygen-injection sparge <br /> wells and five soil gas probes currently are installed. A fee of $2,750.00 (16 x $125 + $375 x <br /> 2) will be required for the well destruction permits.
