ENTRANT INSTRUCTIONS <br /> All personnel who eater confined spaces must be thoroughly familiar with the following duties for entrants as <br /> listed below Your primary responsibilities include <br /> ■ Understand the hazards of the confined space to be entered and the physical effects of those <br />' hazards <br /> ■ Continuously monitor the atmosphere inside of the confined space with a calibrated, direct reading, air <br /> monitoring instrument <br /> ■ Evacuate the confined space <br /> If atmospheric hazards exceed the action level, <br /> If a hazardous condition is identified inside of the confined space, and <br /> • Whenever attendant signals entrants to evacuate <br /> ■ Read and understand the rescue pros-dures <br /> ■ If PPE is required, the entrant must be properly trained on the use of the equipment prior to entry <br /> PIE must be in good working condition <br />' ATTENDANT INSTRUCTIONS <br /> You should be thoroughly familiar with the following dufies when you assume the responsibility of attendant <br /> for a person or persons inside a confined space Your primary responsibilities are <br />' ■ The safety of the personnel inside <br /> ■ Understand the hazards of the confined space to be entered and the physical effects of those <br /> hazards <br /> ■ Maintain the conditions and requirements listed on entry permit <br /> ■ Evacuate the space if you observe any condition which you consider hazardous <br /> ■ Read and understand the rescue procedures Get help if an emergency situation develops never <br /> enter the confined space in an emergency unless you are trained and equipped with the proper <br /> equipment for confined space rescue operations (i e , self contained breathing apparatus, safety <br /> harness, life fine) and are relieved by another attendant <br /> ■ Keep an accurate count of all personnel inside of the confined space at all times <br /> ■ Do not leave the entrance to the confined space while any personnel are still inside unless you are <br /> properly relieved These instructions must be passes onto your relief <br /> v If you have any questions regarding the job, check with your supervisor or a health and safety <br />' rofessianal <br /> ENTRY SUPERVISOR'S INSTRUCTIONS <br /> You should be thoroughly familiar with the following details to quality as the Entry Supervisor for a permit- <br /> required CSE procedure <br /> ■ Requirements for confined space entrant and attendant instructions <br /> ■ Knowledge of the hazards that may be faced during entry including information in the mode, signs <br />' and symptoms and consequences of exposure <br /> ■ Verifies that the appropriate entries have been made on the permit, and that all tests specified by the <br /> permit have been conducted and that all procedures and equipment specified by the permit are in <br /> place before endorsing the permit and allowing entry to begin <br /> ■ Terminate the entry permit when the CSE operations have been complete or when a condition exists <br /> that is not allowed under entry permit requirements <br /> ■ Verifies that rescue services are available and that the means of summoning them are operable <br />' ■ Removes unauthorized individuals who enter or who attempt to enter the permit space during entry <br /> operations <br /> N Responsibility for the confined space when entry is transferred to other personnel <br /> ■ Determines that entry operations are still consistent with the terms of the CSE permit and that the <br /> prescribed intervals regardless of changes in entry personnel <br />' FLUOR DANIEL GTI 90 <br />
