The 205 sensors in all three under dispenser <br /> containments (UDC) need to be changed to 208 sensors. Add the 205 sensor to the transition sump. Add the 208 <br /> sensor to the transition sump shut-off valve. Indicate the location of the 208 sensors in the diesel- and 91-product <br /> submersible turbine pump (STP) sumps. Indicate the location in 344 sensor in the diesel- and 91-product annular <br /> spaces. Indicate the location of the mechanical line leak detectors in the diesel- and 91-product STP sumps. Revise <br /> the legend accordingly. A site plan must be submitted identifying the locations where monitoring will be performed. <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload <br /> a copy of the UST Monitoring Site Plan. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. The Monitoring System <br /> Certification (MSC) Test Report for 3/16/2016 was not found at the facility during the inspection. These records <br /> shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or <br /> sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) <br /> results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring <br /> equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. A <br /> copy of the 2016 MSC Test Report was provided to the facility on the day of the inspection. This was corrected on <br /> site. <br /> This is a Class II violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator failed to complete Item 7 on the Designated Operator Monthly Inspection Report for <br /> 1/2/2017 and 2/11/2017. The designated operator failed to check off that there were two tanks on the 1/2/2017 <br /> report and only indicated one tank when there were two on the 2/11/2017 report. Ensure that designated operators <br /> performing monthly inspections at this facility are including all of the required information on the reports. <br /> This was corrected on site. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 4/15/2017. <br /> Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program <br /> documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST <br /> Certification of Financial Responsibility, UST Response Plan, UST Letter from Chief Financial Officer (if <br /> applicable), and the Owner Statement of Designated Operator Compliance.
