All the required <br /> documentation and/or information necessary to make an eligibility determination was <br /> not submitted . The following documentation and information is missing and must be <br /> submitted to determine your eligibility for placement on the Priority List: <br /> 1 . Several pages of your claim application were missing . Please complete and <br /> submit pages 2 , 31 11 , 12 , 14 , 15 , and 16 . If none of these sections apply, simply <br /> write N/A on the appropriate sections or pages . <br /> 2 . On page 4 of your claim application , please list the product stored in the tanks ; <br /> they must have contained some sort of petroleum substance to be eligible for the <br /> fund . <br /> 3 . On page 5 , number 1 , of your claim application , please provide documentation <br /> from San Joaquin County that substantiates when (date) they first notified you of <br /> the possible existence of the USTs (letter dated 8/5/10 per page 6 of your claim <br /> application) . <br /> Priority Class "B" is for small businesses that are independently owned and operated , <br /> not dominant in their field of operation , employ 100 or fewer employees (including all <br /> affiliates) , and have average annual gross receipts of $ 12 million or less (including all <br /> affiliates) over the three years prior to the date of the claim application . <br /> 4 . If you believe you qualify for Priority Class " B" , please submit the following <br /> documents for review: <br /> A. Complete the enclosed forms : " Request for Assignment of Claim to Priority <br /> Class B" and "Worksheet for Priority Class B Claimants.
