j
Pow
i
MEMO
To: Wes DuBose, Western Meter Service
From: Pam Violett, San Joaquin County Environmental Health
Date: October 7, 1991
Re: Closure/Installation plan for Village West Marina
6649 Embarcadero, Stockton
I have completed an inital review of the above referenced plans,
and I have found several deficiencies. I have enclosed a copy of
Article 3 , sections 2630-2635, of the new California Underground
Storage Tank Regulations that became effective August 1991 to
assist you in understanding the requirements that need to be met
for new UST' s. The following addresses the neccessary resubmittals
needed for each of the plans:
CLOSURE PLAN
1) Obtain and submit an EPA ID#.
2) Submit a copy of your contractor's license and a copy of your
current worker' s comp verification (the one we have on file
expired 10-1-91) .
3) Submit a Health & Safety Plan that is applicable to the tank
removal (not installation) .
4) Enclosed you will find the Authorization to Release that lacks
a date. Please resubmit the form with date.
5) Also enclosed is the tank tracking record. This needs to be
completed to the extent possible prior to removal, i.e. all
blanks filled in except for the date the tanks were removed and
signatures of receiving parties.
6) Submit a site map that shows the existing tanks and piping
runs. If you do not show the underground utilities on the map
you need to state in writing that you will call USA Dig prior
to beginning work.
7) You may need to submit a compaction report. Contact Jerry
Herzick at the San Joaquin County Building Department for
clarification.
