I have enclosed a copy of <br /> Article 3 , sections 2630-2635, of the new California Underground <br /> Storage Tank Regulations that became effective August 1991 to <br /> assist you in understanding the requirements that need to be met <br /> for new UST' s. The following addresses the neccessary resubmittals <br /> needed for each of the plans: <br /> CLOSURE PLAN <br /> 1) Obtain and submit an EPA ID#. <br /> 2) Submit a copy of your contractor's license and a copy of your <br /> current worker' s comp verification (the one we have on file <br /> expired 10-1-91) . <br /> 3) Submit a Health & Safety Plan that is applicable to the tank <br /> removal (not installation) . <br /> 4) Enclosed you will find the Authorization to Release that lacks <br /> a date. Please resubmit the form with date. <br /> 5) Also enclosed is the tank tracking record. This needs to be <br /> completed to the extent possible prior to removal, i.e. all <br /> blanks filled in except for the date the tanks were removed and <br /> signatures of receiving parties. <br /> 6) Submit a site map that shows the existing tanks and piping <br /> runs. If you do not show the underground utilities on the map <br /> you need to state in writing that you will call USA Dig prior <br /> to beginning work. <br /> 7) You may need to submit a compaction report.
