PHS/EHD well permit records should be researched to identify any wells. Identify all <br /> sources of information in the well survey report. Clearly depict each well location as it relates to the above <br /> referenced site and submit any known construction details of the wells. <br /> In addition to the literature search described above, a ground-truth physical survey for wells within a 2,000- <br /> foot radius of the site must also be conducted. The location of wells identified in the physical search must <br /> be provided to PHS/EHD, as well as any known well construction details. All sources of information <br /> utilized in the physical well survey must also be provided to PHS/EHD. <br /> A survey must also be conducted to identify any potential conduits that would allow vertical or lateral <br /> migration of contaminants in groundwater. This survey should also cover a radius of 2,000 feet from the <br /> site, and can be combined with the physical survey for wells. <br /> Continue quarterly groundwater monitoring at the site until otherwise directed by PHS/EHD. Groundwater <br /> samples must be analyzed quarterly for total petroleum hydrocarbons (TPH) as gasoline and TPH as diesel <br /> by EPA Method 8015 ; benzene, toluene, ethylbenzene, and xylenes by EPA Method 8020; oxygenates, 1 ,2 <br /> dichloroethane, ethylene dibromide, methanol and ethanol by EPA Method 8260. All analytical results <br /> must be tabulated in quarterly monitoring reports. Quarterly monitoring reports must be submitted to <br /> PHS/EHD within 30 days of the groundwater-sampling event. <br /> A work plan to determine the lateral and vertical extent of groundwater contamination at this site and a well <br /> survey report must be submitted to PHS/EHD by December 5, 2000.
