PHS/EHD has a <br /> contract with California State Water Resources Control Board to conduct an <br /> underground storage tank program. The above referenced site was placed in the <br /> PHS/EHD Local Oversight Program (LOP) in August 1991. <br /> On June 6, 1991 three UST's were removed under PHD/EHD permit and inspection. <br /> Soil sample results documented a gasoline release and the tank field was <br /> overexcavated to approximately 30 feet below ground surface (bgs). Due to the depth <br /> of the soil contamination and its close proximity to groundwater, a groundwater <br /> investigation was performed to further define the extent of the unauthorized release. <br /> Soil samples taken prior to monitoring well construction and groundwater sample results <br /> indicate a localized low concentration groundwater plume in this area of the facility. <br /> Groundwater monitoring/sampling must continue in this area to demonstrate the release <br /> has a low risk of degrading groundwater quality and is attenuating. <br /> In June of 1996 another area of gasoline contamination was identified approximately <br /> 400 feet north of the 3 former UST's removed in 1991. Bank of Stockton research <br /> discovered information of a former Union Oil 1000 gallon UST that stored gasoline in <br /> this area of the facility. The exact date of the tank closure/removal is unknown to <br /> PHS/EHD. Soil samples from borings and grab groundwater samples have documented <br /> an unauthorized release of gasoline from this former UST. <br /> Geo-Phase Environmental prepared a Work Plan (WP) to perform point source soil <br /> removal and free phase product removal (interim remedial action) and install <br /> groundwater monitoring wells to further define the extent of contamination at the former <br /> Union Oil UST location. The WP was reviewed and approved by PHS/EHD on May 28, <br /> 1997. Pre-approval of corrective action costs (claim #1878) was granted by the State <br /> Water Resources Control Board Underground Storage Tank Cleanup Fund on <br /> November 3, 1997 with provisions. One provision was: you must demonstrate that soil <br /> excavation and disposal is feasible, cost-effective and able to achieve the desired level <br /> of soil cleanup required by PHS/EHD, and Section 2725, Chapter 16, of the <br /> A Division of San Joaquin County Health Care Services <br />
