REVOCABLE PERMIT 2 Ground Water Monitoring Well Permit No . , 3 - 16 Permittee : ARCO PRODUCTS COMPANY /ZZ3 , P . O . BOX 5811 {J OCT 2 6 1993 SAN MATEO , CA 94402 ENVIRONMENTAL HEALTH PERMITISERVICES In accordance with Ordinance 085 - 87 , passed by the Council of the City of Stockton on October 26 , 1987 , a permit revocable at the pleasure of the Public Works Director of the City of Stockton is hereby granted to : ARCO PRODUCTS COMPANY to install and maintain 2 ground water monitoring wells . This permit is granted under the following terms and conditions : 1 . This revocable permit is granted subject to the Standard Conditions Applicable to Revocable Permits adopted by this City Council by Resolution No . 22 , 578 on December 5 , 1960 , and on file in the Office of the City Clerk of the City of Stockton . 2 . The general location of the wells shall be as shown on the map attached . The precise location shall be determined in the field by the City of Stockton . 3 . The Permittee shall keep in full force and effect a Faithful Performance Bond in the amount of $ 1 . 000 . 00 during the life of this permit . 4 . In accordance with Section 1 ( h ) of the " Standard Conditions Applicable to Revocable Permits , " the permittee shall provide and keep in force during the term of the permit a policy of public liability insurance against liability for injuries to persons or property arising out of accidents attributable to the structdre on City property , in amounts to be determined by the Risk Manager . The policy of insurance so provided shall contain a contractual liability endorsement covering the liability assumed by the Permittee by the terms of his permit and shall contain a provision that such policy may not be cancelled except after thirty ( 30 ) days notice in writing to be given to the Risk Manager .
