The most recent secondary <br />containment testing, the routine monitoring system certification reports, and alarm logs for all alarms (some alarms <br />were documented and the following were not: L2 on 7-1-16, L4 on 5-6-16, L5 on 4-9-16) couldn't be located at the <br />time of inspection These records shall be maintained on site for at least three years. Monitoring records include: (1) <br />date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) <br />results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br />readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br />the last three years on site and submit copies to the EHD. <br />This is a CIr-1.— 11 violation. <br />CCR 2712(1) Failure to have an approved UST response plan available onsite. <br />The response plan was not available onsite at file lune of jrispecuu Li�Du u copy is available for inspection at any <br />time. Submit proof of correction to the EHD. <br />This is a Class II violation. <br />CCR 2712 Failure to comply with UST operating permit conditions. <br />The L2 alarm was repaired un i- 1-2016 (per the reviewed onsite maintenance record) and a permit was not obtained <br />from the EHD. Emergency repairs relating to UST system monitoring and release detection may be performed by a <br />properly licensed, trained, and certified contractor pursuant to the EHD'S policy and a permit obtained within one <br />business day from the EHD. All other repairs must have a permit submitted to and approved by the EHD prior to <br />beginning work. Immediately obtain a permit from the EHD for the repaired component . A "Failure to Obtain a <br />Permit" penalty applies. <br />This is a Class II violation. <br />CCR 2715(e) Designated operator monthly inspection reports not maintained on site for at least 12 months. <br />All 2016 designated 0pe1dt01 puas were not found on site. The issue was brought to the <br />attention of Mr. Rattu, who also looked for the missing reports and was unable to locate them. Designated operator <br />monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of <br />the previous twelve months of designated operator monthly inspection reports are maintained on site.
