Valentine Ave <br />Fresno, CA 93722 <br />Re: 7 -Eleven SR0073587 <br />1399 N Main Street, Manteca, CA <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of the <br />application for a permit to replace an underground 87 -octane product pipe between UDC 3/ and the 87 - <br />octane STP sump with OPW FlexWorks dual layer pipe (#C15A) on the underground storage tank (UST) <br />system at the above noted address. According to the permit requestor, the 87 -octane pipe will be <br />removed and replaced through an existing "rock guard" (without concrete breaking), also, no penetrations <br />are scheduled to be replaced as part of this repair. <br />The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any <br />applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Elena Manzo at (209) 953-7699 to arrange the following inspection appointments (a 48- <br />hour notice is required for inspection appointments). <br />a. Piping removal and confirmation soil sampling <br />b. Primary and secondary containment piping pressure test (after surface repairs) <br />c. 24-hour hydrostatic test (or equivalent) on the impacted UDC % and 87 -octane STP sump(s) <br />d. Visual verification of appropriate communication between piping secondary containment and <br />pertinent sensors inside UDC 3/ and 87 -octane STP sumps <br />4. In addition, the following information must be submitted within 30 days after the UST removal <br />date: <br />a. Analytical results: only results that come directly from a third party California certified <br />laboratory with affixed chain of custody will be accepted by EHD <br />b. Provide Piping Hazardous Waste Manifest disposal record <br />c. Hazardous Waste Manifest for rinsate and residual product (if any) <br />5. If concrete will be broken near the tank, a secondary containment test must be performed on the <br />affected diesel UST once concrete is poured in accordance with the manufacturer's guidelines. <br />6.
