("7-Eleven") for the location referenced above.
Item 1: On your inspection report, you noted that the UST certification of
installation/modification was not submitted following the piping replacement performed earlier
this year.
Resolution 1: This update to the CERS system was not completed at the time of your inspection
because the piping replacement was not accepted by Environmental Health. Though the piping
replacement was not accepted, CERS was updated on 9/20/2016 as requested on your inspection
report. Records are currently available in CERS. Note that, 7-Eleven is currently in permitting to
replace the piping installed earlier this year and will update CERS once the piping replacement is
completed.
Item 2: On your inspection report you noted there were three monitoring alarms where
documentation could not be located to confirm they were responded to appropriately.
Resolution 2: The alarms in question were an L2 from 7/1/2016, an L4 from 5/6/2016, and an L5
from 4/9/2016. Documentation of these alarms was attached to the July, June, and April DO
inspections respectively. These inspections and related documents are attached for your records.
Item 3: During your inspection you were unable to locate a copy of the most recent response
plan.
7-Eleven,Inc.
P.O.Box 711—Dallas,Texas 75221-0711
