Well permits are issued by two
different units within EHD, depending on the use of the well/boring, and fees for permits
and inspections differ accordingly. On September 4, 2003, Unit 4 of EHD, issued two
well permits to Kleinfelder for borings at the California Natural Products facility. The
permit fee for each permit was $89, which was paid by Kleinfelder.
For inspections Unit 4 charges an hourly rate ($93/hour). Enclosed is a Master File
Record Form that is to be completed and signed by the appropriate person at California
Natural Products, who is agreeing that California Natural Products will pay our bill for
hourly charges. Please return the completed, signed form to me at the mailing address
shown in the above letterhead. Inspections for the borings must be scheduled,
inspections made and dourly charges paid.
If you have any questions contact me at (209) 468-3449.
Donna Heran, REHS, Director
Environmental Health Department
Margaret Lagorio, Supervising REHS
Unit IV
