Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. The copy in file is dated 4/29/11 and is <br />due by 4/29/12. A new designated operator performed monthly inspection on 10/20/11, 11/29/11, 12/20/11, and <br />1/20/12. Notification was not provided to the EHD within 30 days of the new designated operator <br />change. Any changes shall be reported to the EHD with 30 days. Submit notification to the EHD <br />identifying all the designated operators for this facility within 30 days of receiving this report. <br />201. Alarm log for cold start performed on 3/5/12 was not found on site. These records shall be kept on <br />site for at least 3 years. Immediately locate and maintain the documentation on site and submit copies <br />to the EHD within 30 days of receiving this report. <br />310. Half gallon of liquid was removed from the 87 fill sump. If water could enter into the secondary <br />containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately <br />remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, <br />and manage it accordingly. Ensure that all the sump is maintained free of liquid. Technician removed <br />the liquid and put it in facility's hazardous waste drum. <br />Notes <br />Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br />each violation was corrected, including supporting documentationto the EHD within 30 days of receiving <br />this report. <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br />Inspector.
