I have attached an employee certificate issued <br />by Environ prior to them being taken over by OPW, and then the certificates from OPW. The Environ "T" in Disp #3-4 is cracked at the port hole. Also Rick reviewed with me <br />that the "T's in the UDCs do not drain. Only one drain spot for the system and it is in a sump. I have attached pictures of the "T" <br />failing a test. <br />The work order previously sent to you should have had only the recent notes to avoid confusion. During our first visit to test the <br />facility the ports did not have plugs. We returned to the site with plugs and tested and discovered the leak AT the port in the "T", <br />that is what's in the photo. So that's why the permit is to replace the cracked "T".
