Main Street, Manteca, CA <br /> Dear Mr. Naidu: <br /> Per the inspection report dated 12/01/08, enclosed are the documents requested for the <br /> above referenced site. Specifically enclosed are: <br /> •A copy of the service ticket from Walton Engineering for the clean out of all <br /> sumps. <br /> •A copy of the Designated Operator Owner Statement form listing Gilbarco <br /> Veeder-Root and Ryan Wyatt as the Designated Operator. <br /> •A copy of the Designated Operator Owner Statement from listing Belshire <br /> Environmental Services Inc. (Belshire) and Brian Hernandez as the Designated <br /> Operator. Belshire was contracted to perform the Designated Operator monthly <br /> inspections starting 01/01/09. <br /> •A copy of the Training Log and the 12/08/08 monthly DO inspection report, <br /> which states employee Tarlochan Singh was trained. <br /> •A copy of updated forms including Business Owner/Operator, Operating Permit <br /> Application-Facility Information, Tank Information, UST Monitoring Plan, and <br /> UST Response Plan. <br /> The inspection report alleges service tickets for alarms on 1/4/08 and 1/6/08 were not <br /> available. 7-Eleven is investigating the response status with Fuel Quest, the remote <br /> monitoring contractor and a follow up letter will be submitted with supporting <br /> documentation. <br /> Should you have additional questions or concerns please do not hesitate to contact me <br /> at (916) 463-6776. <br />
