Levand Family Trust
47 East 11 st Street
Tracy, California 95376
Page 2 of 2
November 25, 2014
Decontaminate all sampling tools with an Alconox solution and clean all probing rods
prior to starting each boring;
Containerize any soil or water waste generated during field activities in properly labeled
55 -gallon drums; and
Notify the EHD at least 48 hours prior to conducting grouting procedures.
The EHD approves the work plan with the following conditions:
• Soil from the tank pit was found to be slightly elevated for lead. Add analysis for
dissolved lead for grab ground water samples (shallow) from the tank pit and the boring
immediately north of the former UST area;
• During abandonment of boreholes deeper than 30 feet bsg (the maximum free -fall
depth) and/or boreholes containing groundwater, use a tremie pipe extending to within 5
feet of the borehole's total depth to pump the cement mixture upward from the bottom of
the boring; and
• Notify the EHD at least 2 weeks prior to implementing fieldwork so that an inspector can
schedule to be present for the well destructions; and
The EHD requires that work begin no sooner than 5 working days after the EHD permits
have been issued.
To proceed with obtaining the permits for these borings, the following must be submitted to the
EHD to the attention of Gabriel Gonzalez:
Two EHD Well and Boring Permit applications (one for each parcel/address);
Master File Record form;
Licensed Contractor's Declaration (LCD); and
A fee of $650 ($390 x 1 + $130 x 2). The permit fee for borings per parcel is $130, along
with a $390 Inspection Activity fee.
This EHD approval is limited to only the methods proposed in the work plan for boring
advancement and abandonment, soil and groundwater sampling, equipment decontamination,
waste management, and technical justification for the proposed work. It is not a concurrence or
endorsement for any other conclusions, conditions or implications contained in the AGE
Preliminary Site Assessment Work Plan, dated 24 September 2014.
Please contact Gabriel Gonzalez at (209) 468-3442, or by email at gagonzalez@sjcehd.com if
you have any questions regarding this letter.
Sincerely,
Gabriel Gonzalez Nuel Henderson, PG
Environmental Health Specialist Engineering Geologist
c: Brian Millman, PG, Advanced GeoEnvironmental, Inc., 837 Shaw Road, Stockton, CA
95215
ec: GeoTracker
James L.L.
