The following is an itemized list of hazardous materials violations that have not been addressed for MECCA EQUIPMENT as of October30, 2019. Open violations from August 13, 2019 inspection Violation#6 - Failed to complete and/or submit a site map with all required content. The map does not show/address internal roads, adjacent streets, storm and sewer drains, access and exit points (from the street), emergency shutoffs, hazardous material handling and storage areas(containers of oil in the storage room attached to the parts room on the south west end of the hanger and the drums of caustic soda located at the men's bathroom), and emergency response equipment(fire extinguishers and spill control). A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as "NA". Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #9 - Failed to annually review and certify that the business plan is complete, accurate, and up to date. The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS) annually for by January 15th for 2019. The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. Employee training records were not available at the time of inspection. The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
