(AGE)has prepared this Monitoring Well Destruction Report-4th Quarter 2009 for 3230 North <br /> West Lane, (site), Stockton, California. A total of five (5) ground water monitoring wells (1\4W-1 <br /> through MW-5R) were destroyed under San Joaquin County Environmental Health Department <br /> (EHD) oversight. The site setting is illustrated on Figure 1. Site structures and monitoring well <br /> destruction locations are depicted on Figure 2. <br /> INTRODUCTIONBACKGROUND <br /> Ground water monitoring wells MW-1 through MW-5R were previously installed at the site as part <br /> of a leaking underground storage tank(UST)investigation.Well MW-1 was installed to a total depth <br /> of 60 feet below surface grade (bgs). Wells MW-2 and MW-3 were installed to a total depth of <br /> 55 feet bsg, and MW-4 and MW-5R were installed to depths of 69.5 and 70 feet bsg,respectively. <br /> All wells were single completed with 2-inch PVC casing to their respective depths. <br /> In a letter dated 08 October 2009, the California Regional Water Quality Control Board - Central <br /> Valley Region concurred with the EHD recommendation to close the site, and further directed that <br /> wells MW-1 through MW-5R be properly destroyed. <br /> PROCEDURES <br /> On 20 November 2009,five(5)ground water monitoring wells were destroyed at the site under EHD <br /> 837 Shaw Road,Stockton,California 95215 <br /> Telephone (209)467-1006 Fax(209)467-1118 <br />
