Also enclosed with this <br /> work plan is a well destruction permit and the associated well destruction permit fee. <br /> INTRODUCTIONBACKGROUND <br /> To date,a total of five ground water monitoring wells have been installed onsite. On 05 and 06 June <br /> 2000,three ground wate ,moni o me-,�isSMW-1 MW-2 and MW-3)were installed at the site to <br /> maximum depths of 60 feilel sg uttii Zi&diameter PVC well casing. On 24 and 25 October <br /> 2005, pilot borings MW-4 and MW-5 were completed as ground water monitoring wells using <br /> two-inch diameter PVC casing with 0.020-inch slotted screen installed from approximately 60-70 <br /> feet bsg. <br /> Due to grout intrusion in MW-5 as detailed in the AGE-prepared Proposal to Re-Install Ground <br /> Water Monitoring Well MW-5 dated 22 December 2005,AGE plans to install MW-5R onsite. EHD <br /> approved this proposal by letter dated 06 January 2006,and further directed AGE to properly destroy <br /> MW-5 immediately. The procedures outlined below will be applicable for the current destruction <br /> of MW-5 as well as for the rest of the on-site wells(MW-1 through MW-4)at a later date when they <br /> are no longer in use. <br /> In order to ensure that ground water monitoring wells do not provide a vertical conduit for <br /> contaminants, AGE proposes to properly destroy them in accordance with San Joaquin County <br /> Environmental Health Department(EHD)Standards For Well Construction and Destruction. <br /> 837 Shaw Road, Stockton,California 95215 <br /> Telephone(209)467-1006 Fax(209)467-1118 <br />
