Nt,r <br />2535 Wigwam Drive <br />Stockton, CA 95205 <br />Re: Waterloo Food Mart, 4315 Waterloo Road, Stockton, CA SR0060935 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the AMENDED application for a permit to replace the gasket on the 87 fill sump lid and replace <br />the penetration fittings on all four UDCs and vent sump on the underground storage tank (UST) <br />system at the above noted address. The permit has been approved with the following <br />conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Visual inspection of UDCs after interstitial fluid refill or equivalent test. Inspection <br />must be done before backfill of the excavation. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. A valid, current California UST Service Technician certificate issued by the <br />International Code Council (ICC), and <br />c. A valid, current certificate of training from the manufacturer(s) of the component(s) <br />being tested, repaired, or serviced. <br />
