REHS <br />Jeff Carruesco, REHS, RDI <br />Kasey Foley, REHS <br />Linda Turkatte, REHS <br />SR0060935 <br />The San Joaquin County Environmental Health Department (EHD) has completed the review of <br />the application for a permit to replace the gasket on the 87 fill sump lid and replace the <br />penetration fittings on all four UDCs on the underground storage tank (UST) system at the <br />above noted address. The permit has been approved with the following conditions: <br />Contact the local fire department and the San Joaquin Valley Air Pollution Control <br />District for any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval <br />prior to commencing work. All new addenda supersede the previous ones. <br />3. Contact Garrett Backus at (209) 468-2986 to arrange the following inspection <br />appointments (a 48-hour notice is required for inspection appointments). <br />a. Secondary piping vacuum test or equivalent. Test the product and vapor recovery <br />piping. One hour vacuum test per manufacturer's guidelines. If there are no <br />manufacturer's guidelines, test shall be performed using an applicable method <br />specified in an industry code or engineering standard. Inspection must be done <br />before backfill of the excavation. <br />b. Visual inspection of UDCs after interstitial fluid refill or equivalent test. Inspection <br />must be done before backfill of the excavation. <br />Please be aware that the facility cannot operate the UST system prior to the EHD <br />inspection and approval. <br />4. Qualified service technicians must possess or work under the direct and personal <br />supervision of an individual physically present at the work site who possesses all of the <br />following: <br />a. One of several permissible valid, current contractor's licenses issued by the <br />Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />
