Financial responsibility documents were obtained from the facility today. These financial <br /> responsibility documents are incomplete. Current financial responsibility documents are required to be <br /> submitted annually. Immediately submit a corrected copy of the financial responsibility documents to the <br /> San Joaquin County Environmental Health Department (EHD). Alternatively, log into CERS, upload the <br /> required information, and submit for review by the EHD. Complete by November 2, 2013. <br /> 201. Complete alarm logs were not found on site. The following alarms were not documented in the <br /> alarm log by the facility manager or clerk: S2 - vapor line - 7/11/13, 5/29/13 - communication alarm; <br /> S3 - 91 line - 9/6/13, 6/17/13 - no vacuum alarm; S3 - 91 line - 7/3/13 - communication alarm., S4 - <br /> vent lines - 8/28/13 - communication alarm. Document all L, Q and S alarms in the alarm log. Include <br /> the date of the alarm, identification of the alarm sensor, and the action taken to resolve the alarm. <br /> Complete a corrective action statement and submit to the EHD by November 2, 2013. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> designated operator monthly inspection reports and failed to check that they were responded to <br /> appropriately. The missing alarms include: S1 - 87 line - 5/13/13 - high liquid alarm, S1 - 87 line - <br /> 5/20/13 - no vacuum alarm, S2 - vapor line - 7/11/13, 5/29/13 - communication alarm; S3 - 91 line - <br /> 9/6/13, 6/17/13 - no vacuum alarm; S3 - 91 line - 7/3/13 - communication alarm., S4 - vent lines - <br /> 8/28/13 - communication alarm, L18 - vent annular sensor - 10/21/12, 10/13/12, 10/4/12 - fuel alarm. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous <br /> month, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br /> designated operators performing monthly inspections at this facility are including all of the required <br /> information on the reports.
