_MWI, MW2, MW3,MW4, MW10 and MW13 were all situated on the station property owned by Lincoln Properties An encroachment agreement with Lincoln Properties was active throughout the investigation remediation and restoration of this site MW6 and MW9 were situated in the asphalt parking area north and west of the station An encroachment agreement with the property owner, Fahn Properties was obtained Well MW7 is on San Joaquin County easement A encroachment/demo ht ion permit was obtained from San Joaquin County Public Works Wells MWS and MWS are on City of Stockton easements An encroachment pernut was obtained from the City of Stocktkon Well MW11 is on property owned by Signature Theaters Mr George Mann of Signature Theaters signed the encroachment agreement on August 1, 2000 Well MWI2 is on property owned by Dr Sidney Kerston of 6257 Meadowstone Drive, Santa Rosa, California 95409 Dr Kerston signed the encroachment agreement on July 1, 2000 EXCAVATED WELL MATERIALS AND FLUIDS All materials (asphalt, concrete, monitor wells materials, soils and PVC pipe) excavated/removed during well and environmental system lateral destructions was placed inside the treatment compound in a debris pile All fluids generated from the destruction of the wells and equipment decon was mixed with the soils generated from well destruction's and placed into the debris pile, no drummed water existed A four point composite sample analyzed for TPHg, BTEX, MtBE and lead was obtained to document the soil condition prior to removal This debris pile (20 67 tons) was removed and disposed of at Forward Landfill by Manley & Sons Trucking on October 29, 2001, see Appendix C for profile and Appendix D for Disposal weight tags No fluids were disposed of to the sanitary sewer, the final monthly wastewater discharge update dated October 23, 2001 terminated the sewer discharge permit, see Appendix E UST FUND PRE-APPROVAL Pre-approval for costs to destroy the monitoring wells and environmental system laterals was obtained, see Appendix F LIMITATIONS The information presented in this report is based on the following 1 The observations and data collected by field personnel 2 The result of laboratory analyzes performed by a state certified analytical laboratory 3 Our understanding of the regulations of San Joaquin County, the City of Stockton and the State of California 4 References reviewed for this report 4 R-603 site restoration 11-2001
