The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution <br />Control District for any applicable permits and requirements. <br />2. Contact Toua Yang at (209) 468-3459 to arrange the following inspections <br />appointments (a 48-hour notice is required for inspection appointment(s): <br />a. No additional testing is required at this time for sensors. EHD was onsite <br />at the time of installation. The new sensors were tested and deemed <br />functional. <br />b. Contact EHD to verify sump repair and witness sump pressure test. <br />3. Any changes in information and scope of work must be submitted to EHD for <br />approval prior to commencing work. All new addenda supersede the previous <br />ones. <br />4. All components installed must be certified by an independent testing organization <br />and bear the appropriate mark as required by the independent testing organization. <br />5. Qualified installers and service technicians must possess or work under the direct <br />and personal supervision of an individual physically present at the work site who <br />possesses all of the following: <br />a. One of several permissible valid, current contractor's licenses issued by <br />the Contractors State License Board (may be issued to the installation <br />company), and <br />b. A valid, current California UST Service Technician certificate issued by <br />the International Code Council (ICC), and <br />c. A valid, current California UST Installation/Retrofitting certificate issued <br />by the International Code Council (ICC), and <br />
