Box 1025 <br /> West Sacramento, CA 95691 <br /> Fax: (916) 373-1172 <br /> Subject: Waterloo Shell,4315 E Waterloo Road,Stockton,CA(SR0057395) <br /> The San Joaquin County Environmental Health Department (EHD) has completed the review of <br /> the amended application for a permit to repair/replace the 87 tank STP sump and under dispenser <br /> containment(UDC) 3/4. The sump was fiberglassed on the outside and the UDC Blue Line <br /> penetrations were resealed with Blue Line glue. The permit has been approved with the <br /> following conditions: <br /> Conditions of approval: <br /> 1. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD) for approval prior to commencing work. All new addenda supercede <br /> the previous ones. <br /> 3. Contact Garrett Backus to arrange the following inspection(s) (a 48 hour notice is <br /> required): <br /> a. A soap test to verify the UDC and sump interstitial space integrity. Isolate and <br /> pressurize the pipe following the manufacturer's guidelines. <br /> 4. A"Certification of Installation/Modification" form shall be submitted by the owner or <br /> the owners'agent [pursuant to(CCR)2635(d)] before this upgrade will be considered <br /> complete by EHD. <br /> 5. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API, ASTM, etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 6. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> a. One of several permissible valid, current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company), and <br /> b. All valid, current California UST certificates issued by the International Code <br /> Council (ICC), that are applicable to the work being performed, and <br /> Page 1 of 2 <br />
